How to Request Your ChexSystems Report

- Complete the online form with your full name, address, date of birth, and Social Security number.
- Request by Phone:
- Call 1-800-428-9623 (available 24/7) to request your report.
- Request by Mail:
- Download the Consumer Request for Disclosure Form.
